Care Coordinator

Description:

POSITION Overview:

 Works with chronically ill or high‐risk patients and their family/caregiver(s) to coordinate care
 Collaborates and refers patients to other providers of healthcare and community services
 Completes monitoring and follow up via phone or home visits in order to coordinate services
 Connects patients with community resources
 Assists patients with accessing preventative care
Qualifications

 Required Bachelor’s Degree, LPN, RN, from an accredited university required.
 Bachelor’s Degree in any area will be considered with work experience in community resource or

clinical setting
 Preferred 3‐5 years’ experience in clinical or community resource settings; care coordination and/or

case management experience
 Preferred knowledge about and connections to community health care and social welfare resources is
