EvoHealthMD is seeking Certified Wound Care Nurse Practitioners (NPs) dedicated to making a meaningful difference in patient care. Providing advanced wound care in patients’ homes and senior living communities across Indianapolis and surrounding areas, these NPs utilize innovative therapies such as tissue regenerative treatments, UltraMist Therapy, and advanced dressings to promote faster healing, reduce hospitalizations, and improve quality of life. As part of a compassionate, mobile care team, Nurse Practitioners play a vital role in transforming lives.
Clinical Duties & Patient Visits
- Conduct thorough, compassionate in-person evaluations at patient homes, assisted living, and long-term care facilities
- Perform detailed wound assessments, including staging and documentation
- Develop and update individualized wound care plans
- Administer advanced treatments such as tissue regenerative therapies (e.g., amniotic and allograft grafts), UltraMist Therapy, advanced dressings, and sharp or mechanical debridement to support medical necessity and promote healing
- Monitor wound healing and adjust care plans as needed
- Collaborate with primary care providers, home health agencies, and interdisciplinary team members to ensure coordinated care
- Educate patients and caregivers on wound care and prevention strategies
Documentation
- Complete timely, HIPAA-compliant documentation within 48 hours using the EHR
- Maintain accurate records to support care coordination and compliance
Technology, Equipment & Contractor Responsibilities
- Independent contractor (1099) role; active licensure and malpractice insurance required
- Contractors provide their own devices and printer/scanner for documentation
- Proficient with wound care software (Wound Expert, Tissue Analytics) and wound photography
- Maintain HIPAA compliance and data security on personal equipment
- Reliable internet required for timely documentation uploads
- EvoHealthMD provides software access and mileage reimbursement at the state rate
Qualifications
- Active Nurse Practitioner license (FNP or AGNP preferred)
- Wound care certification (WCC®, CWS®, CWCN®) or relevant wound care experience
- Strong clinical judgment and independent decision-making skills
- Must relocate to Indianapolis, IN before starting
- Reliable transportation required
