Case Management Nurse

 

Description:

Nebraska Medicine is seeking a Case Management Nurse to join their Surgical Oncology team in Omaha, NE. This role focuses on coordinating patient care throughout the surgical process, providing education and advocacy, facilitating discharge planning and support services, assisting with insurance and resource needs, and collaborating with the multidisciplinary team to ensure safe, seamless, and holistic care.


Key Responsibilities

  • Manage patient care delivery for a designated group of patients to achieve high-quality, cost-effective outcomes across the healthcare continuum.

  • Coordinate surgical oncology patient care from admission through discharge.

  • Provide patient education, support, and advocacy throughout the care journey.

  • Assist with discharge planning and access to support services.

  • Collaborate with physicians, nurses, and other interdisciplinary team members to ensure comprehensive patient care.

  • Evaluate patient care needs, make independent clinical judgments, and prioritize interventions.


Required Qualifications

  • Bachelor’s Degree in Nursing (BSN)

  • Current Registered Nurse license in Nebraska or compact state license

  • Minimum of 3 years nursing experience

  • Effective verbal and written communication skills

  • Ability to prioritize and make independent clinical judgments

  • Experience with basic patient care equipment, policy, procedures, and protocols

  • Ability to work collaboratively with an interdisciplinary team

  • Effective critical thinking and analytical skills

  • Ability to evaluate care across the continuum

  • Basic Life Support (BLS) certification from AHA-endorsed program

  • Pediatric Advanced Life Support (PALS) certification as department dependent


Preferred Qualifications

  • Master’s of Science in Nursing (MSN) degree

  • Experience in oncology, cardiovascular care, NICU, nephrology, solid organ transplantation, medical/surgical care, emergency care, critical care, med spa, dialysis, or transplant coordination

  • Knowledge of healthcare financial environment and reimbursement systems

  • Experience with Microsoft Office, graphic software, patient teaching materials, and patient registries/databases

  • Leadership or team supervision experience

  • Preferred certifications: ACLS, Blood and Marrow Transplant Certified Nurse, Certified Diabetes Educator (CDE), IBCLC, Oncology Nursing Certification (OCN), Chemotherapy/Biotherapy, Trauma Nursing Core Course (TNCC)


Shift Details

  • First Shift: Monday – Friday, 8:00 AM – 5:00 PM

  • Full-time, 1.0 FTE (40 hours per week)
