Description:
Oklahoma State University’s Center for Health Sciences is seeking a Clinical Case Manager to join its multidisciplinary healthcare team in Tulsa, OK. This grant-funded position is dedicated to providing comprehensive case management and care coordination for patients, with a strong focus on HIV primary care services.
The Clinical Case Manager plays a pivotal role in assessing, planning, implementing, coordinating, and monitoring healthcare options and services to ensure quality, cost-effective outcomes. This position requires a compassionate, detail-oriented professional capable of working collaboratively with patients, healthcare providers, and community partners to address health needs and remove barriers to care.
Key Responsibilities:
Assess, plan, and coordinate individualized care services for patients to ensure quality health outcomes.
Facilitate access to core medical services and coordinate HIV primary care service delivery.
Develop and implement a Comprehensive Medical and Health-related Services Care Coordination Plan in collaboration with patients and healthcare teams.
Provide treatment adherence readiness counseling, patient education, and support for treatment compliance.
Monitor and evaluate patient progress, update care plans, and reassess patient needs regularly.
Document all care contacts, interventions, and progress accurately.
Serve as an advocate for patients, facilitating access to social services, referrals, and resources.
Promote patient engagement by addressing barriers such as housing, substance use, incarceration, or social stigma.
Collaborate with community resources to ensure comprehensive support for patients and families.
Participate in multidisciplinary case conferences and team discussions to ensure coordinated care.
Required Qualifications:
Master’s degree in Counseling Psychology (MS), Social Work (MSW), or a BSN in Nursing. Equivalent educational/experience combinations may be considered.
For RNs: Must hold a valid professional nursing license issued by the Oklahoma State Board of Nurse Registration and Education.
Strong interpersonal and communication skills.
Proficient written planning and documentation skills.
Knowledge of local healthcare and social service systems, eligibility assistance, and case coordination.
Ability to work effectively with vulnerable populations, including rural communities, communities of color, LGBTQ+ individuals, and individuals impacted by substance use, incarceration, or homelessness.
Local travel within the 918 area of Oklahoma required; must have reliable transportation.
Preferred Qualifications:
Licensure in Counseling or Social Work (LCSW or LPC).
2–5 years of professional experience in a healthcare or human services setting.
Prior experience in case management.
Bilingual (English/Spanish) skills.
Work Schedule & Appointment:
Monday to Friday, 8:00 am – 5:00 pm.
Regular, continuous appointment (grant-funded).
