Description:

DayBreak Adult Care is seeking a dedicated Clinical Case Manager to join its mission-driven team in San Leandro, CA. This role is central to providing CalAIM Enhanced Care Management (ECM) services, ensuring that high-risk and high-acuity members receive holistic, coordinated care that improves their health and well-being.

The Clinical Case Manager will act as the primary contact for referrals, engage members within 48 hours, conduct comprehensive assessments, and design actionable, client-centered care plans. The role requires close collaboration with healthcare providers, caregivers, social workers, and community resources to reduce hospitalizations, prevent institutionalization, and enhance quality of life.

Key Responsibilities include:

  • Conducting initial and ongoing assessments and creating tailored care plans.

  • Coordinating care across physicians, specialists, caregivers, and community resources.

  • Providing health promotion and transitional care to support long-term stability.

  • Utilizing motivational interviewing and problem-solving to encourage positive behavior change.

  • Responding to urgent client needs by arranging emergency supports such as food, transportation, or temporary housing.

  • Maintaining compliance with HIPAA, DHCS, and Elder Abuse Reporting requirements through accurate documentation.

  • Leading and mentoring peers, supporting program operations, and ensuring adherence to ECM requirements.

  • Engaging in organizational events, trainings, audits, and cross-program collaborations.

Qualifications & Skills:

  • Strong communication, assessment, and care coordination skills.

  • Knowledge of Alameda County resources and social service networks.

  • Proficiency in documentation, data management, and technology tools.

  • Ability to work independently, prioritize tasks, and thrive in a collaborative environment.

  • Awareness of health disparities and cultural competence in serving diverse populations, including low-income seniors, BIPOC, LGBTQ+, and individuals with disabilities.

Other Requirements:

  • Current/Active California Driver’s License and ability to travel within Alameda County.

  • Ability to manage physical tasks including lifting items up to 25 lbs (e.g., walkers, wheelchairs, boxes).
