Description:

The Director of ISP Development and Quality provides agency-wide leadership over goal development, documentation, tracking, and evaluation within the Individualized Service Plan (ISP) framework. This role ensures that assessments are translated into meaningful, measurable, person-centered goals and that Therap is used consistently and with integrity to monitor progress and outcomes. By establishing standards and systems (not managing daily

operations) this position strengthens service quality, accountability, and regulatory readiness across all programs.

Core Areas of Focus
 

  • Meaningful Goal Development (Within ISPs)
  • Lead agency-wide standards for assessment-driven, person-centered goal development
  • within the ISP framework
  • Translate assessments and interdisciplinary input into clear, measurable, functional goals
  • Chair and steward the Goal Development Committee, ensuring high-quality, consistent goal practices across programs
  • Define expectations for goal measurability, relevance, and alignment to assessed needs
  • Guide goal revisions when progress data indicates limited effectiveness
  • Consistent Therap Entry & System Integrity
  • Serve as the agency’s lead for Therap integration and use related to goal documentation and tracking
  • Establish and maintain standards for:
  • Goal structure and naming conventions
  • Documentation workflows tied to goals
  • Data accuracy, consistency, and completeness
  • Ensure uniform use of Therap for goal entry, tracking, and outcome documentation across all programs
  • Provide oversight and guidance to maintain system integrity (without direct operational management)
  • Outcome Management & Reporting
  • Oversee systems for monitoring goal progress and determining goal outcomes
  • Analyze outcome data to identify trends, gaps, and improvement opportunities
  • Lead the agency’s outcomes reporting framework to support:
  • Quality improvement efforts
  • Licensing and regulatory readiness
  • Leadership review and decision-making
  • Ensure outcome data is actively used to inform goal refinement and service planning
     

Key Responsibilities
 

  • Provide agency-wide leadership for goal development and outcome evaluation within ISPs
  • Establish and enforce standards for goal quality, progress measurement, and revision
  • Monitor goal-level data to strengthen consistency and effectiveness across programs
  • Partner with program leadership, QA, and clinical teams to strengthen goal-setting practices
  • Serve as the central steward of Therap-based goal and outcomes systems
