Family Consultant

 

Description:

ESSENTIAL JOB FUNCTIONS: The role of the Family Consultant is to provide support to unpaid family caregivers using a method that empowers caregivers to overcome challenges of caregiving and establish a system of support that will sustain them through their caregiving journey.

 

Under the supervision of the Family Services Supervisor, the Family Consultant:

  • Provides consultation to caregivers on the phone or in person.
  • Conducts intake requests and performs comprehensive assessments.
  • Creates Action Plan and coordinates services for caregivers.
  • Completes thorough and accurate chart documentation.
  • Provides educational material and resources to caregivers.
  • Facilitates support groups and educational classes.
  • Completes other work assignments as directed by Program Supervisor.

 

EMPLOYMENT STANDARDS:

 

  • Bachelor’s degree in social work, gerontology, health services, or related field.
  • At least 2 years professional experience working with older adults.
  • Strong communication skills, both written and spoken, with strength in active listening.
  • Thorough understanding of the needs of family caregivers and long-term-care issues.
  • Extensive knowledge of community resources, with a focus on services for older adults.
  • Willingness to travel occasionally within 10-county service area.
