Health Home Care Manager

Doral Health & Wellness, a leading multi-specialty healthcare organization in New York State, is currently hiring a Health Home Care Manager to join its expanding Care Management team. The organization is dedicated to delivering integrated and compassionate care to individuals with chronic medical, behavioral health, and developmental challenges, particularly those enrolled in Medicaid.

This full-time, hybrid role includes a blend of remote work, office responsibilities, and in-person field visits throughout Brooklyn and surrounding communities. The Care Manager will be responsible for creating, coordinating, and updating person-centered care plans tailored to the diverse needs of clients with complex health conditions.

Key Responsibilities:

• Conduct comprehensive assessments and develop individualized care plans.

• Coordinate care across physical, behavioral, and social service sectors.

• Support clients in navigating healthcare systems, securing housing, attending appointments, and accessing entitlements.

• Promote self-management strategies for medical and mental health conditions.

• Maintain timely, accurate documentation within EHR systems.

• Ensure compliance with NYS Health Home program guidelines and performance standards.

• Collaborate with clients' families, community organizations, and healthcare providers.

Required Qualifications (per NYS DOH):

• Bachelor’s degree in a health/human services field plus 2 years of relevant experience
  OR

• Master’s degree in a health/human services field plus 1 year of experience
  OR

• Licensed Registered Nurse (RN) plus 2 years of experience

Additional Requirements:

• Familiarity with Medicaid populations, chronic illnesses, and social determinants of health.

• Strong communication, case coordination, and organizational skills.

• Proficiency in Microsoft Office and EHR platforms.

• Ability to conduct field visits in Brooklyn and nearby areas.

• Bilingual proficiency in Spanish, Haitian Creole, Bengali, or other languages is highly desirable.

Compensation & Benefits:

• Salary: $55,000 annually (equivalent to $4583/month; dependent on experience)
