The Yukon-Kuskokwim Health Corporation (YKHC) is a leading Tribal health care organization serving nearly 30,000 residents across rural southwest Alaska. With Bethel as the regional hub, YKHC provides healthcare to communities in one of the most remote and culturally diverse regions of the United States.

As a Health Management Specialist, you will play a critical role in protecting public health by connecting individuals with preventative services, chronic care management, and infectious disease prevention support. This role focuses on health education, disease prevention, and referral to essential medical and social services. Specialists must follow strict protocols to gather health information, provide education, and ensure timely connections to testing, treatment, or other resources.

This is a remote, temporary, full-time position (40 hours per week) requiring strong communication, cultural awareness, and organizational skills. The position often requires evening and weekend availability to respond to urgent cases and may also include assisting with administrative tasks during low caseload periods.

Key Responsibilities:

  • Conduct outreach to individuals requiring preventive care, chronic care management, or infectious disease follow-up.

  • Provide culturally sensitive health education, guidance, and referrals for medical or social services.

  • Collect and manage health-related data accurately while maintaining strict confidentiality and HIPAA compliance.

  • Follow standard scripts and protocols for patient interviews and communication.

  • Collaborate with health professionals and community resources to ensure continuity of care.

  • Assist with additional department needs such as supply management, phone support, or administrative tasks.

Qualifications:

  • Education: High school diploma or GED required; Associate degree preferred.

  • Experience:

    • Required: Lived or worked in the Yukon-Kuskokwim Delta region.

    • Preferred: Experience in healthcare, public health, community outreach, education, or social services. Experience with data entry, interviews, and Microsoft Office applications.

  • Skills:

    • Strong verbal and written communication skills with cultural sensitivity.

    • Proficiency with computers, mobile devices, and applications.

    • Organizational and multitasking abilities with strong time management.

    • Ability to work independently and collaboratively in a multidisciplinary team.

    • Ability to communicate in Yupik or Athabascan highly desirable.

  • Other Requirements:

    • Completion of a contact tracing training module within 3 months of hire.

    • HIPAA-compliant home office setup, including reliable internet, phone, and computer capable of remote system access.
