Licensed Practical Nurse

Description:

Actalent is urgently hiring Licensed Practical Nurses to provide direct, comprehensive home healthcare services to patients in their homes across New York City boroughs. This role involves one-on-one patient care under the supervision of a registered nurse or physician, focusing on medication administration, clinical assessments, and patient education within regulatory guidelines.

Key Responsibilities:

• Conduct comprehensive patient health assessments

• Administer medications, treatments, and therapies as prescribed

• Monitor vital signs and report abnormalities to healthcare providers

• Assist patients with activities of daily living such as bathing, grooming, dressing, and toileting

• Provide wound care, injections, and skilled nursing services as needed

• Educate patients and families on health management, medication administration, and disease prevention

• Maintain accurate patient records documenting assessments and interventions

• Collaborate with healthcare team members to ensure continuity of care

• Comply with regulatory policies and procedures governing home healthcare

Essential Skills & Requirements:

• Valid New York State LPN license

• Current CPR certification from AHA or RedCross

• Strong clinical assessment and critical thinking skills

• Comfortable working independently with patients in their homes

• Willingness to work 12-hour shifts, minimum 3 days per week, including weekdays and weekends

• Willingness to travel and work across multiple boroughs in NYC

• Reliable transportation

• Willing to work for a maximum pay of $40 per hour
