Licensed Practical Nurse

Description:

Sodus Rehab & Nursing Center is seeking Licensed Practical Nurses (LPNs) to provide safe, high-quality care for residents in a long-term care environment. This role requires administering medications, performing treatments, assisting with daily care, and communicating effectively with residents and healthcare teams. Key Responsibilities:

• Provide resident care according to established standards and policies.

• Administer medications accurately following the five rights of medication administration.

• Perform treatments using sterile or clean techniques and external feedings.

• Communicate resident information and plan of care during shift changes and transfers.

• Participate in quality improvement processes for resident care.

• Complete care activities within scheduled shift times.

• Report and delegate resident care issues to appropriate nursing management personnel.

• Adapt to reassignments in the interest of resident care.

Qualifications:

• Licensed Practical Nurse (LPN) certification required.

• Minimum 1 year of experience; new graduates welcome.

• Ability to work Evening/Night shifts and every other weekend.

• Knowledge of age-specific needs, nursing procedures, and treatments.

• Competent in medication administration, patient care, and documentation.

Working Environment:

• May require use of safety equipment, including HEPA masks.

• Exposure to blood, body fluids, or tissue (OSHA Category I).

• Work performed at a single facility location.
