Key Responsibilities:

• Deliver individualized nursing care to patients in their homes and care facilities.

• Monitor, record, and report patient health status to physicians and supervising nurses.

• Administer prescribed medications and treatments as directed by physicians.

• Educate patients and their families on health maintenance and disease prevention.

• Maintain accurate and detailed patient records in accordance with healthcare regulations.

• Collaborate with interdisciplinary healthcare teams to develop and implement care plans.

• Ensure compliance with safety, infection control, and ethical standards in patient care.

Requirements:

• High school diploma or GED required

• Current State License as a Licensed Practical Nurse (LPN)

• Minimum of one year of relevant nursing experience

• Excellent communication, organizational, and computer skills

• Valid driver’s license, auto insurance, and reliable transportation
