Elara Caring, one of the nation’s leading providers of home health care, hospice, and personal care services, is seeking a Licensed Practical Nurse (LPN) – Home Health PRN to join their dedicated healthcare team in Kirksville, Missouri. As part of Elara Caring’s mission — delivering the Right Care, at the Right Time, in the Right Place — you will be responsible for providing quality nursing care, assisting with patient assessments, and supporting the development and implementation of individualized care plans. You will work closely with registered nurses, physicians, and other healthcare professionals to ensure patients receive the highest standard of care possible.

Key Responsibilities:

• Provide skilled nursing care and support to patients in their homes according to physician orders and care plans.

• Monitor, document, and report patient progress, symptoms, and reactions to treatments or medications.

• Administer prescribed medications and treatments while adhering to all clinical protocols and safety standards.

• Support the assessment of patient needs and assist in developing individualized plans of care.

• Collaborate effectively with interdisciplinary team members to ensure holistic patient care.

• Educate patients and families on care techniques, medication management, and lifestyle adjustments.

• Maintain accurate, detailed clinical documentation for each patient encounter.

• Travel to patients’ homes reliably to perform assigned visits.

Qualifications and Requirements:

• Current and valid LPN/LVN license in the state of Missouri (or eligibility for reciprocity).

• Minimum 1 year of experience in an acute care or equivalent clinical environment.

• Current CPR certification through the American Heart Association or American Red Cross.

• IV certification preferred.

• Excellent communication and interpersonal skills with a compassionate and positive attitude.

• Strong dedication to quality patient care and adherence to safety standards.

• Reliable transportation for home visits.
