Nurse Practitioner

Description:

Matrix Medical Network provides clinical services focused on primary care and managing at-risk health plan members in their homes. The Nurse Practitioner role involves performing comprehensive health risk assessments in various community settings, including home visits, skilled nursing facilities, and via telehealth.

During each visit (lasting up to one hour), providers review medical history, medication adherence, social environment, and other health risks to identify chronic conditions and improve care quality. This role allows new graduate NPs to apply as well.

Key Responsibilities:

• Conduct adult and geriatric assessments including diagnosis, treatment, health education, physician and case management referrals, and clear documentation according to Matrix protocols.

• Collaborate with physicians, case managers, social workers, family members, caregivers, and ancillary medical staff as needed.

• Coordinate with Primary Care Physicians for patient education and follow-up.

• Provide care in multiple venues: home visits, skilled nursing facilities, and telehealth.

Schedule: Flexible day, evening, night, weekend shifts available.

Required Qualifications:

• Master’s degree or commensurate experience and successful NP licensure completion.

• Current RN and NP licensure in state of practice with prescriptive authority or ability to obtain it.

• Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty.

• Current BLS, ACLS, or CPR certification.

• Minimum 1 year experience as Nurse Practitioner.

• Strong computer skills and familiarity with employee health/medical record software.

• Excellent verbal and written communication skills.

• Comfortable and flexible with frequent changes.

Travel Requirements:

• Ability to travel to patient homes and facilities.

• Valid driver’s license and adequate automobile insurance for state of residence.
