Matrix Medical Network is seeking PRN Nurse Practitioners to perform in-home risk assessments for adult and geriatric populations across Converse, Texas, and surrounding communities. As a Nurse Practitioner, you will conduct comprehensive health evaluations that go beyond routine office visits. These 60-minute visits involve assessing patients’ health conditions, medications, lifestyle, and social environments. Your insights will support the primary care team by closing care gaps, identifying chronic conditions, and improving health outcomes.

Key Responsibilities:

• Conduct in-home or skilled facility visits to complete adult/geriatric risk assessments

• Evaluate patient medical history, medications, and health education needs

• Identify chronic conditions and recommend follow-up or referrals

• Document findings according to Matrix guidelines and protocols

• Collaborate with primary care physicians, social workers, and other medical staff

• Provide assessments in diverse settings including homes, SNFs, and community locations

Compensation & Work Schedule:

• Monthly Pay: $2000 – $10000 based on 20–100 completed visits

• Pay Per Visit: $100 per completed assessment

• Schedule: Flexible hours – daytime, evenings, weekends (optional)

• Employment Type: PRN / Part-Time / Remote

• Work Type: No on-call required unless desired

Required Qualifications:

• Master's Degree or equivalent experience with NP licensure

• Active RN and NP license in the state of practice

• Board Certification (AANP, ANCC, or AACCN approved by Matrix)

• BLS, ACLS, or CPR certification

• Strong written and verbal communication skills

• Tech-savvy with proficiency in medical software and documentation

• Valid driver’s license and automobile insurance

• Willingness to travel within assigned territory
