Nurse Practitioner

Mitchell Martin Inc. is actively seeking a Nurse Practitioner to provide direct and indirect clinical care to participants in a healthcare clinic and day center setting located in Oceanport, NJ. This is a full-time, on-site role on the day shift and is ideal for a candidate with a passion for geriatric care and a commitment to interdisciplinary collaboration.

As a Nurse Practitioner, you will work under the supervision of the Medical Director and manage complex clinical scenarios, providing both preventive and responsive care. This includes conducting health assessments, managing chronic and acute conditions, ordering diagnostic tests, and initiating appropriate interventions. You will also play a critical role in participant and family education and coordination with the broader healthcare team.

Key Responsibilities:

• Conduct health histories and physical exams

• Order, interpret, and follow up on diagnostic tests and lab work

• Manage and adjust medication and treatment plans

• Provide emergency response and recognize abnormal clinical signs

• Document care activities in the electronic medical record (EMR)

• Coordinate care plans with interdisciplinary team members

• Refer participants to specialists or other services as required

• Serve as a liaison for participants, families, and clinical staff

• Precept and mentor medical students and residents

• Evaluate and treat patients across multiple care settings, including clinics and long-term care environments

Qualifications:

• Master’s degree in clinical nursing from an accredited program

• Current RN licensure and Family Nurse Practitioner certification

• 3–5 years of nursing experience; prior Nurse Practitioner experience preferred

• Experience with elderly, frail, or long-term care populations strongly preferred

• Valid driver's license and reliable transportation

• Medically cleared for communicable diseases with up-to-date immunizations
