Matrix Medical Network provides clinical services that improve the health of at-risk members by offering home risk assessments and medical evaluations. As a Nurse Practitioner (NP) - PRN in the Home Risk Assessments team, you will conduct comprehensive assessments of adults and geriatric patients, reviewing medical histories, diagnoses, treatments, and medication adherence. Your role will help identify chronic conditions, improve the quality of care, and close care gaps, all while working in patients' homes, skilled nursing facilities, and other community settings.

You will collaborate with physicians, case managers, and caregivers to ensure appropriate follow-up and documentation. This position offers flexible scheduling, including days, evenings, nights, and weekends.

Key Responsibilities:

• Conduct Adult/Geriatric assessments, including medical history, diagnoses, treatments, and health education.

• Provide clear documentation and follow Matrix guidelines.

• Collaborate with healthcare teams, including Primary Care Physicians (PCPs) and social workers, to ensure patient care.

• Perform services in a variety of settings: home visits, skilled nursing facility visits, and other community environments.

• Provide education and follow-up with patients and caregivers.

Requirements:

• Master’s Degree or equivalent experience with NP licensure and prescription authority (or ability to obtain).

• Board-certified by AANP, ANCC, or AACCN in an approved specialty.

• Current BLS, ACLS, or CPR certification.

• 1 year of experience as a Nurse Practitioner (preferred); new grads welcome.

• Strong computer skills and familiarity with medical record software.

• Excellent verbal and written communication skills.

• Ability to adapt to frequent changes and travel for patient visits.

Travel Requirements:

• Valid state driver’s license, ability to drive, and proof of automobile insurance.

• Travel to meet patients at their homes or care facilities is required.
