Matrix Medical Network, a pioneer in in-home clinical healthcare services, is seeking dedicated Nurse Practitioners or Physician Assistants to join their team in a PRN (per diem) capacity. This role offers the opportunity to make a direct and meaningful impact on patients’ lives by performing comprehensive health assessments in the comfort of their homes or within skilled nursing facilities.

As part of Matrix’s nationwide network of clinicians, you will be at the forefront of patient-centered care, conducting detailed assessments that provide critical insights into members’ overall health and well-being. These assessments are designed to bridge care gaps, improve health outcomes, and enhance the quality of life for elderly and at-risk individuals who may not receive consistent primary care.

Key Responsibilities

  • Conduct comprehensive adult and geriatric health assessments including medical history, diagnoses, treatment plans, and health education.

  • Document all findings accurately according to Matrix Medical Network’s clinical guidelines.

  • Administer and order point-of-care or lab-based tests as necessary.

  • Collaborate with primary care physicians, case managers, and caregivers to ensure coordinated care.

  • Provide clear recommendations for follow-up, specialist referrals, or case management when necessary.

  • Deliver assessments at patients’ residences — either in private homes or skilled nursing facilities.

  • Support the goal of closing multiple care gaps in a single visit through comprehensive assessment and documentation.

  • Competitive Pay: Earnings are based on the number of assessments completed. Providers completing 20–100 visits monthly typically earn between $1,800–$11,500 per month, depending on state rates ($90–$115 per visit).

Qualifications

  • Master’s Degree or equivalent professional experience.

  • Current NP or PA licensure in state of practice, with prescriptive authority (or ability to obtain).

  • Board certification by NCCPA, AANP, ANCC, or AACCN in relevant specialties (Adult, Family, Emergency, or Acute Care).

  • Current BLS, ACLS, or CPR certification.

  • At least 1 year of experience preferred; new graduates are welcome.

  • Strong computer literacy, communication, and clinical documentation skills.

  • Valid driver’s license and proof of adequate automobile insurance.
