Nurse Practitioner

Matrix Medical Network is seeking compassionate and skilled Nurse Practitioners for PRN Home Risk Assessments. This role is designed to improve patient outcomes by conducting comprehensive health assessments in members’ homes or community settings, focusing on at-risk individuals including older adults. New graduate Nurse Practitioners are welcome to apply.

Key Responsibilities:

• Conduct adult and geriatric health assessments, including medical history review, diagnosis, treatment, health education, and referral coordination following Matrix guidelines

• Collaborate with primary care physicians, case managers, social workers, caregivers, and other medical personnel to optimize patient care

• Perform visits at patient homes, skilled nursing facilities, and other community settings

• Provide clear and timely documentation for all assessments and follow-up care

• Engage in patient education and case management referrals to close care gaps

• Adapt to frequent changes in scheduling and patient needs with flexibility

Qualifications:

• Master’s degree in Nursing or equivalent experience with NP licensure

• Current RN and Nurse Practitioner licensure in the state of practice with prescriptive authority or ability to obtain it

• Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty

• Current BLS, ACLS, or CPR certification

• Minimum 1 year of Nurse Practitioner experience

• Strong computer skills and familiarity with electronic health/medical record software

• Excellent verbal and written communication skills for interacting with patients, families, and clinical teams

• Valid driver’s license, ability to travel locally, and proof of automobile insurance as required

• Comfortable working independently and in a flexible, changing environment

Compensation and Benefits:

• Pay is $90 per completed assessment, with monthly earnings ranging from approximately $1,800 to $9,000 depending on visits completed

• PRN providers are non-benefited except for 401(k) and limited voluntary benefits

• Mileage reimbursement plan for travel

Work Schedule:

• Part-time PRN position with flexible scheduling available

• Day shifts primarily, Monday to Friday, with optional evenings, nights, or weekends

• No on-call duties unless desired
