Nurse Practitioner

Description:

HealthFair, through Matrix Medical Network, is offering a PRN Nurse Practitioner position to perform home risk assessments and clinical evaluations. The role involves conducting comprehensive adult and geriatric health assessments including medical history review, diagnosis, treatment, health education, physician and case management referrals, and documentation in accordance with Matrix guidelines. This position supports primary care providers by delivering care where patients live, including home visits and skilled nursing facilities.

The role is flexible with scheduling options across days, evenings, nights, and weekends, and requires travel to patient locations. Compensation is performance-based per completed visit. Benefits include 401(k) and limited voluntary options, with mileage reimbursement.

Key Responsibilities:

• Conduct adult and geriatric health assessments (medical history, diagnosis, treatment)

• Provide health education, referrals, and follow-up services

• Collaborate with physicians, case managers, social workers, family members, and other healthcare personnel

• Deliver services in varied settings: home visits, skilled nursing facilities, community environments

• Document visits clearly per organizational protocols

Qualifications:

Required:

• Master’s Degree or equivalent experience with successful NP licensure

• Current RN and NP licensure in the state of practice, including prescriptive authority or ability to obtain it

• Board certification by AANP, ANCC, or AACCN in an approved specialty

• Current BLS, ACLS, or CPR certification

• Minimum 1 year NP clinical experience

• Strong computer skills and familiarity with medical record software

• Excellent verbal and written communication skills

• Ability to adapt to frequent change

• Valid driver’s license, ability to drive, and proof of adequate auto insurance

Work Hours and Schedule:

• PRN (as needed) with flexible scheduling options including days, evenings, nights, weekends

• Travel required to patient locations
