Optimum Connect Health Services, under the brand RoadL, is expanding its innovative Fee-for-Service in-home healthcare model and is actively seeking compassionate and skilled Nurse Practitioners to join their team. This role involves conducting in-home visits primarily in New York’s boroughs including Queens, as well as Suffolk and Nassau Counties. The position offers a flexible work environment with meaningful impact on patient lives.

The Nurse Practitioner will be responsible for comprehensive patient assessments, developing and implementing personalized care plans, managing both chronic and acute medical conditions, and providing high-quality care in patients’ homes. Collaboration with other healthcare professionals is essential to ensure optimal patient outcomes. Accurate and timely documentation is also required.

Key Responsibilities:

  • Conduct thorough assessments and evaluations of patients in their homes.

  • Develop and implement individualized care plans based on clinical assessments.

  • Manage and stabilize chronic and acute medical conditions effectively.

  • Deliver compassionate and professional medical care and support to patients and their families.

  • Collaborate with healthcare teams to coordinate patient care and optimize outcomes.

  • Maintain detailed documentation of patient interactions and care activities.

Qualifications:

  • Valid Nurse Practitioner license in New York State.

  • Board certification mandatory.

  • Enrollment as a Medicaid provider required.

  • Preferred prior experience in home healthcare or related clinical settings.

  • Strong clinical assessment, critical thinking, and decision-making abilities.

  • Excellent communication and interpersonal skills.

  • Ability to work independently with effective time management.

Compensation:

  • Competitive fee-for-service payment model.

  • Earn up to $200 per patient visit, depending on visit type.

  • Rate applicable through December 31, 2024.
