Nurse Practitioner Supervisor

Mitchell Martin Inc. is seeking a qualified and experienced Nurse Practitioner Supervisor to join its healthcare team in Brooklyn, NY. This full-time, on-site position offers a meaningful opportunity to provide high-quality care to a diverse adult patient population while leading and coordinating a team of healthcare professionals.

The Nurse Practitioner Supervisor will collaborate closely with primary care physicians to ensure comprehensive, coordinated patient care. The successful candidate will be responsible for clinical duties including diagnostic assessments, chronic illness management, and wellness visits, as well as for overseeing care planning and documentation accuracy.

Work Schedule:

• Full-time, Day Shift

• Monday to Friday

• 08:30 AM to 05:00 PM

Key Responsibilities:

• See adult patients and perform physical exams, diagnostic tests, and routine assessments

• Conduct annual wellness visits and quarterly evaluations

• Manage acute and chronic conditions with tailored treatment plans

• Provide same-day acute care services when needed

• Conduct timely post-hospitalization visits and facilitate smooth care transitions

• Educate patients and families about prevention and health promotion

• Ensure complete and accurate documentation, using appropriate diagnosis codes

• Lead care planning and supervise delivery for community-based participants

• Participate in ongoing quality improvement programs

Required Qualifications:

• Graduate of an accredited Nurse Practitioner program

• Licensed as both a Nurse Practitioner and Registered Nurse in the state of New York

• DEA Certification and Board Certification required

• Minimum of 2 years in a clinical primary care setting

• At least 1 year experience working with adults or appropriate training

• Strong interpersonal and communication skills

• Proficiency in basic computer and office software

Compensation:

• Annual Salary: USD 155000 – USD 165000

• Monthly Equivalent: USD 12916 – USD 13750
