Description:
The Medical College of Wisconsin (MCW) is seeking a highly skilled and compassionate Advanced Practice Provider (APP)—either a Physician Assistant (PA) or Nurse Practitioner (NP)—to join the Division of Geriatric and Palliative Care Medicine. This part-time position is based at Froedtert Menomonee Falls Hospital and Froedtert West Bend Hospital, where the APP will provide comprehensive inpatient palliative care services.
In this role, the APP will collaborate with palliative care physicians, interdisciplinary team members, patients, and families to deliver holistic, patient-centered care. The focus is on symptom management, end-of-life care, patient safety, and supporting families during critical health transitions. The APP will contribute not only to clinical practice but also to education, quality improvement, and program development in palliative and hospice care.
Key Responsibilities:
Collaborate with interdisciplinary teams to diagnose, treat, and manage complex medical conditions in the inpatient environment.
Conduct comprehensive patient histories and physical examinations, assessing physical, cognitive, and psychosocial health.
Order, perform, and interpret diagnostic tests and screenings, ensuring timely communication and documentation of results.
Develop and monitor both pharmaceutical and non-pharmaceutical treatment plans tailored to patient needs.
Write progress notes, admission and transfer orders, and maintain accurate documentation in the Electronic Medical Record (EMR).
Facilitate care coordination and communication among patients, families, and healthcare teams to ensure continuity of care.
Perform palliative care-related procedures as credentialed by the institution.
Provide education and guidance on symptom management, pain management, and end-of-life skills to nursing staff, residents, fellows, medical students, and home care/hospice staff.
Act as a preceptor/educator for APN students and contribute to committee work and continuous quality improvement efforts.
Stay updated on current developments and trends in NP/PA practice and palliative care.
Position Requirements:
Nurse Practitioner:
Master’s degree or higher in Nursing.
Wisconsin RN License and Wisconsin Advanced Practice Nurse Prescriber License.
Certification as Adult Care NP or Geriatric NP preferred (Acute Care NP or Family NP also considered).
Additional certification in Hospice/Palliative Care or Oncology Nursing preferred.
AHA ACLS, AHA BLS, DEA required.
Physician Assistant:
Bachelor’s degree or higher in Physician Assistant Studies.
Current Wisconsin PA License.
Certification by NCCPA.
AHA ACLS, AHA BLS, DEA required.
Preferred Experience:
Minimum of 1–2 years in a clinical or hospital setting with palliative care, hospice care, or hematology/oncology patients.
5 years of overall experience as an APP preferred.
Work Schedule:
Primarily Monday–Friday, 8:00 am – 4:30 pm, with flexibility to cover additional hours depending on patient and business needs.
