• Provide comprehensive outpatient primary care to seniors with a focus on continuity and quality of care.

• Manage small patient panel sizes, enabling physicians to develop meaningful relationships with patients.

• Focus on improving health outcomes for elderly patients and those with multiple chronic conditions.

Ideal Candidate Profile:

• Board certified/eligible in Internal Medicine or Family Medicine.

• Geriatrics fellowship training is welcomed but not mandatory.

• Demonstrates a passion for senior care, chronic disease management, and serving underserved communities.
