Description:
Actalent is hiring a Registered Nurse to join as a PACE Community Health Nurse (PCHN) under the direction of a Nurse Manager. The PCHN provides skilled nursing care and routine clinical nursing assessments to PACE members and their families, ensuring compliance with laws, regulations, and agency policies. The role involves being part of an interdisciplinary team (IDT) and managing assigned members in accordance with CMS PACE regulatory requirements.
Responsibilities include:
Implementing the agency’s mission, philosophy, and policies
Taking responsibility for assignments and performing within education and experience limits
Complying with nursing laws and providing care through all nursing process elements
Participating in mandatory PACE comprehensive care planning meetings
Collaborating with interdisciplinary team members to develop personalized care plans
Essential Skills:
Case management, patient care, and care management expertise
Valid New York State Registered Nurse license
Physical and mental stamina to perform nursing duties
Valid driver’s license and vehicle for work-related travel
Additional Skills & Qualifications:
Legal authorization to practice nursing in NY State
Medically cleared for communicable diseases with up-to-date immunizations
Computer proficiency, organizational and communication skills
Ability to work independently with supervision, and manage ancillary personnel
Ability to navigate stairs, walk between multiple sites during workday
Work Environment & Schedule:
Community Health Centers in Bronx and Bushwick
Monday to Friday, 8:30 am to 5:00 pm, 37.5 hours/week, with potential weekend work
Fully onsite position
Pay and Benefits:
Hourly pay between $50 and $55
