HealthCare Support is currently seeking a highly qualified Manager of Care Delivery – RN to join a respected healthcare organization in Queens, New York. This leadership role is ideal for a seasoned registered nurse with strong clinical, administrative, and team management experience, especially within the home care sector.

In this full-time, on-site position, the Manager of Care Delivery will oversee a team of 4 to 5 community nurses. The responsibilities include coordinating patient care activities, managing staffing and resources, ensuring accurate documentation, conducting investigations when necessary, and ensuring strict compliance with regulatory standards.

The work schedule is Monday to Friday, 8:30 AM to 5:00 PM, with the flexibility of one remote workday per week available after the first 90 days of employment.

Required Qualifications:

• Active New York State Registered Nursing (RN) license

• 5 years of clinical nursing experience

• Proven leadership experience

• At least 3 years of home care experience

Preferred Qualifications:

• Bachelor of Science in Nursing (BSN)

• Completion of NYSED-approved infection control coursework

• Prior experience with managed long-term care insurance is a plus

• Case Management Certification is recommended within the first two years of employment
