Rn Case Manager

Description:

Carilion Clinic, a regional healthcare leader based in Roanoke, Virginia, is seeking an experienced and compassionate RN Case Manager – Hospice to join its New River Valley team. The role involves delivering exceptional hospice and palliative care to patients across diverse settings, including homes, hospitals, nursing homes, and adult care centers. This is a full-time position with a Monday–Friday schedule (8:00 AM – 4:30 PM) and on-call rotations, offering a sign-on bonus of up to $30,000 along with mileage reimbursement, company equipment, and safety provisions.

The RN Case Manager will coordinate and manage individualized care plans, delegate and oversee care delivery, and provide end-of-life support with sensitivity to physical, emotional, and spiritual needs. The role requires excellent clinical judgment, critical thinking, and the ability to multitask in fast-paced environments.

Key Responsibilities:

• Develop, coordinate, and manage patient care plans in collaboration with patients, families, and healthcare teams.

• Conduct patient visits across homes, hospitals, and other care facilities.

• Provide supervision for Hospice LPNs, CNAs, volunteers, and students.

• Engage in end-of-life discussions with compassion and sensitivity.

• Maintain patient confidentiality and uphold Carilion Clinic’s Mission, Values, and Guiding Principles.

• Handle routine issues independently and escalate complex matters when necessary.

• Participate in on-call rotations and provide care in adverse weather or other Carilion locations as needed.

Qualifications and Requirements:

• Graduate of an accredited Registered Nurse Program.

• Current Virginia RN license or eligibility for Virginia licensure.

• Valid Virginia driver’s license (for Home Health) and ability to pass a Criminal Record Check as per state law.

• AHA BLS-HCP certification (must obtain/maintain).

• One (1) year of hospice, home health, oncology, or medical-surgical nursing experience preferred.

• Strong keyboarding skills for EMR systems; ability to meet essential job functions.
