Service Coordinator

Services for the UnderServed (S:US) is seeking a compassionate and driven Service Coordinator to join its team in Amityville, NY. This full-time, on-site position plays a critical role within the Supportive Services for Veteran Families (SSVF) program, directly assisting veterans and their families in securing stable, permanent housing through Rapid Rehousing and Homelessness Prevention efforts.

The Service Coordinator acts as the primary case manager for veterans, developing and maintaining Housing Stability Plans, connecting clients with essential services, and guiding them through complex human services systems. This position involves frequent fieldwork (up to 40% of the workweek), engaging clients in their communities, shelters, homes, and service centers.

Key responsibilities include:

• Delivering core SSVF services and housing support to clients

• Providing person-centered, trauma-informed case management using a progressive engagement model

• Conducting client assessments to determine needs, strengths, and service goals

• Navigating veterans through VA systems, public benefits, and housing programs

• Coordinating with landlords, community partners, and service agencies to ensure comprehensive support

• Assisting clients in accessing employment resources, health care, and other supportive services

• Managing and documenting Temporary Financial Assistance with internal teams and third-party vendors

• Ensuring all service documentation and compliance standards are met

The successful candidate must have a deep commitment to supporting underserved populations, particularly veterans, with a strong understanding of housing barriers and the skills to navigate systemic challenges. The ability to work independently, organize caseloads efficiently, and build trust with clients is essential.
