ArchWell Health, an innovative senior healthcare provider, is seeking a Licensed Social Worker to join its Shared Market Clinical team in Holiday, Florida. This role focuses on delivering high-quality social services to senior members, helping them overcome socioeconomic and medical barriers while fostering a sense of community and wellness.

Key responsibilities include conducting social services assessments, coordinating and managing support packages, navigating managed care plans, providing case management for social and behavioral care, and assisting members with access to prescription programs and benefits. The role also involves proactive identification of process improvements and supporting care transitions.

The ideal candidate must hold a BSW or MSW license (Master’s preferred) in the state of practice, possess strong interpersonal and active listening skills, and demonstrate a mission-driven mindset with a focus on senior care.
