Social Worker

Puget Sound Kidney Centers is seeking a compassionate and qualified Social Worker to join its patient care team. The role is dedicated to helping patients with chronic kidney disease adjust to dialysis treatment and maximize their psychosocial well-being. The Social Worker will play a vital part in patient education, advocacy, and ongoing support, working closely with medical staff to ensure holistic care.

The successful candidate will demonstrate strong interpersonal skills, the ability to work with diverse populations, and a commitment to community-focused healthcare. Responsibilities include assessing patient needs, facilitating psychosocial adjustment, developing individualized care plans, and connecting patients with resources that improve their quality of life. For candidates still working towards LICSW, supervised hours will be provided free of cost.

This role requires flexibility, as work may involve travel between PSKC’s Smokey Point and Monroe clinics, with occasional visits to other facilities based on patient and operational needs.
