Description:

Memorial Healthcare System is seeking a Social Worker with an MSW to provide comprehensive psychosocial care for patients and families, ensuring their well-being and improving their overall quality of life. The role involves performing bio-psychosocial assessments, interventions, and treatment while collaborating closely with an interdisciplinary healthcare team. The Social Worker identifies high-risk psychosocial factors affecting patient care and supports both patients and staff in navigating these challenges effectively.

Key Responsibilities:

  • Conduct crisis interventions, including Baker Act when necessary.

  • Collaborate with governmental agencies (DCF, APS, CMS, KePro) to ensure safe post-acute care.

  • Participate in interdisciplinary care conferences and consult with nursing staff to evaluate ongoing patient needs.

  • Assist with discharge planning, patient transfers, and community coordination.

  • Guide patients and families in navigating healthcare systems, insurance plans, and post-acute care options.

  • Provide social work services in psychological functioning, adaptation to illness/injury, psychosocial assessment, and facilitation of community resources.

  • Identify and intervene in suspected abuse cases, including children, adults, and seniors.

  • Document services in the electronic medical record and demonstrate proficiency with EMR systems.

  • Accountability, decision-making, crisis intervention, customer service, patient-centered care, problem-solving, adaptability, and adherence to professional standards.

Education & Certification Requirements:

  • Master’s degree in Social Work (MSW) – Required

  • BLS certification as required

Working Conditions & Physical Requirements:

  • Work is primarily on-site with frequent standing, walking, reaching, and repetitive hand/arm movements.

  • Exposure to biohazardous materials, bodily fluids, dust, latex, moving mechanical parts, needles/sharp objects, and potential physical assault.

  • Ability to perform office tasks including computer work and documentation.

Shift: Full-time, primarily day hours (office workers)
