Nurse Practitioner

Description:

Matrix Medical Network is seeking compassionate and skilled Nurse Practitioners (PRN) to join their national in-home clinical network. The role involves performing comprehensive health assessments for patients in their homes or Skilled Nursing Facilities, providing personalized care, and collaborating with physicians, case managers, and caregivers to close care gaps and improve patient outcomes.

This is a remote, flexible role allowing providers to schedule visits based on personal availability.

Key Responsibilities

• Conduct adult/geriatric health assessments including medical history review, diagnosis, treatment, and health education.

• Administer and order point-of-care and lab-based testing as needed.

• Collaborate with Primary Care Physicians (PCPs) and care teams on patient education and follow-up.

• Provide care in patient residences, including homes and Skilled Nursing Facilities.

• Document all patient interactions clearly and accurately according to Matrix guidelines.

• Support closing multiple care gaps in a single visit through the Matrix Comprehensive Health Assessment (CHA).

Qualifications & Requirements

• Master’s Degree in Nursing or equivalent experience with NP licensure.

• Current NP licensure in the state of practice, with prescription authority or ability to obtain it.

• Board certification (AANP, AACN, or ANCC) in Adult, Adult Gerontology Primary Care, Family, Emergency, or Acute Care specialty.

• Current BLS, ACLS, or CPR certification.

• Valid driver’s license and adequate automobile insurance for the state of residence.

• Strong computer skills and familiarity with electronic health record systems.

• Excellent verbal and written communication skills.

• 1 year of NP experience preferred; new graduates are encouraged to apply.
