Nurse Practitioner

Zephyr Behavioral Health and Wellness is looking for a dedicated In-Home Care Nurse Practitioner (NP) to provide primary care services to individuals living in psychiatric sober living homes. This role focuses on managing medically complex patients with co-occurring psychiatric conditions, performing health assessments, managing medications, and conducting lab draws when needed. The position offers flexible scheduling with a minimum commitment of one day per week.

The NP will collaborate closely with psychiatric providers to implement holistic treatment plans and monitor patient progress. They will be responsible for documenting progress in the Electronic Medical Record (EMR) system and communicating with home staff and family members as appropriate.

Key Responsibilities:

• Conduct in-home visits to client homes (minimum of 1 day per week).

• Provide primary care services such as health assessments, medication management, and lab draws.

• Collaborate with psychiatric providers to ensure holistic treatment plans.

• Monitor patient progress and document in EMR.

• Communicate with staff and family to ensure continuity of care.

• Respond to medical needs and provide follow-up care as needed.

Qualifications:

• Licensed Nurse Practitioner (FNP, AGNP, or similar) in Colorado.

• Experience working with high-risk psychiatric populations is preferred.

• Comfortable performing lab draws and conducting in-home visits.

• Independent, self-directed with strong communication skills.

• Must commit to at least 1 day per week of in-home care visits.

• Valid driver’s license and reliable transportation are required.

Compensation & Benefits:

• 70/30 revenue split (you receive 70% of the revenue).

• Estimated hourly rate: $100–$140/hour based on CPT code reimbursement and visit volume.
