Nurse Practitioner

Description:

Matrix Medical Network is seeking qualified and compassionate Nurse Practitioners (PRN) to perform Home Risk Assessments in Mount Joy Township, PA. This role is ideal for both experienced NPs and recent graduates looking to gain hands-on clinical exposure in the community. Providers will deliver services primarily in home and skilled nursing facility environments.

This role focuses on conducting comprehensive health assessments which typically last up to one hour per visit. During these visits, practitioners evaluate the member’s medical history, current health status, medication adherence, and social and environmental risks. The objective is to close multiple care gaps in a single encounter while supporting improved health outcomes.

You will work closely with physicians, case managers, social workers, and other healthcare personnel. Flexibility is a key component, with the ability to choose shifts during day, evening, or weekend hours—no mandatory on-call or overnight shifts required.

Key Responsibilities:

• Perform comprehensive geriatric/adult medical assessments

• Collaborate with PCPs and case management teams for patient education and follow-up

• Document clinical findings according to Matrix guidelines

• Provide services across multiple settings including homes and skilled nursing facilities

• Identify chronic conditions, health education needs, and care coordination requirements

Key Requirements:

• Master's degree in Nursing or equivalent experience with NP licensure

• Active RN and NP license in Pennsylvania with or eligibility for prescriptive authority

• Board certification through AANP, ANCC, or AACCN in an approved specialty

• CPR/BLS certification

• Strong communication skills and adaptability to change

• Valid driver's license with reliable transportation and insurance

Compensation & Benefits:

• PRN providers earn $100 per completed visit

• Monthly earnings range from $2000 to $10000 depending on volume
