FOX Rehabilitation is actively expanding its team and looking for top-caliber Physical Therapists to deliver high-quality geriatric care through our renowned Geriatric House Calls™ model in St. Joseph, MI and surrounding locations. This position offers the unique opportunity to provide 1:1 therapy services in patients’ homes, focusing on function, independence, and quality of life.

As a FOX Physical Therapist, you will work autonomously, manage your own treatment schedule, and be part of a clinician-led, interdisciplinary team. FOX fosters an environment that supports professional growth, encourages innovation, and values its clinicians' contributions.

Key Responsibilities:

• Provide physical therapy services in a 1:1 setting to older adults in their homes.

• Develop and implement personalized, functional care plans based on patient goals.

• Collaborate with occupational and speech therapists to ensure interdisciplinary treatment.

• Use clinical expertise and judgment to drive rewarding patient outcomes.

• Maintain accurate documentation and clinical records.

• Promote patient-centered care with a focus on abolishing ageism and maximizing independence.

What You'll Need:

• A valid Physical Therapy license in the state of Michigan (or eligibility to obtain one)

• A degree from an accredited physical therapy program

• Basic computer literacy

• Current CPR certification
