Description:

Elara Caring is seeking a dedicated and compassionate Physical Therapist Assistant (PTA) to join its expanding home health team in Ann Arbor, MI. This role requires a professional who is passionate about delivering personalized care and committed to maintaining the highest standards of physical therapy practice. You will work under the supervision of a Physical Therapist, directly providing therapy services, educating patients and families, and ensuring continuity of care in accordance with state and federal guidelines.

Key Responsibilities:

  • Deliver direct physical therapy services as prescribed by a licensed Physical Therapist and certified by the patient’s physician.

  • Follow the Plan of Care (POC), ensuring accurate and effective implementation tailored to each patient’s needs.

  • Assess patients’ progress and communicate any status changes to the supervising Physical Therapist.

  • Instruct patients and caregivers on the safe and effective use of prosthetics, orthotics, mobility aids, and assistive devices (e.g., walkers, wheelchairs, canes).

  • Provide patient and family education for therapy home programs to support rehabilitation and independence.

  • Adhere strictly to state practice act guidelines and Elara Caring’s mission and policies.

  • Perform physically demanding tasks, including lifting, handling, and transferring patients (up to 100 lbs.).

  • Travel extensively (up to 50%) within the branch coverage area as required.

Qualifications & Requirements:

  • Graduate of an accredited Physical Therapy Assistant program approved by the American Therapy Association.

  • Current, unrestricted PTA license valid in the state of Michigan.

  • Minimum of 6 months of PTA experience preferred.

  • Strong physical ability to meet the demands of patient care.

  • Willingness and ability to travel with reliable transportation, valid driver’s license, and up-to-date auto insurance.
