Professional Case Management (PCM), a leading provider of in-home healthcare services, is seeking dedicated and compassionate Registered Nurses (RNs) to deliver exceptional care to clients who have served our country. This position offers the flexibility to create your own schedule while making a meaningful difference in the lives of patients in the comfort of their homes.

As a Home Health RN, you’ll play a key role in assessing patient needs, developing care plans, performing skilled nursing interventions, and collaborating with multidisciplinary teams to ensure high-quality care. This role is ideal for nurses who value one-on-one patient relationships, professional independence, and flexible weekend schedules.

Key Responsibilities:

• Perform comprehensive patient assessments and document findings accurately.

• Administer medications and IV therapy as prescribed, ensuring safety and compliance.

• Develop individualized care plans focused on achieving realistic health outcomes.

• Provide direct nursing care and delegate responsibilities as appropriate based on staff competence.

• Educate patients and their families on health management, treatment plans, and self-care techniques.

• Monitor and evaluate patient responses, adjusting care plans as needed.

• Act as a patient advocate, promoting dignity, comfort, and safety at all times.

• Collaborate with physicians, case managers, and other healthcare professionals to coordinate care.

• Maintain detailed clinical notes and documentation in compliance with agency standards.

• Promote infection control and safety practices in patient homes.

• Participate in team meetings, professional training, and ongoing skill development.

Qualifications & Requirements:

• Graduate of a state-approved school of professional nursing (BSN preferred).

• Active and unrestricted RN license in the state of Tennessee (or applicable practice state).

• Current CPR certification and recent TB test.

• Reliable means of transportation for home visits.

• Strong communication, assessment, and organizational skills.

• Commitment to maintaining confidentiality and professional standards.

Work Schedule:

• Part-time / PRN availability (Saturday and Sunday only).

• Day and night shifts available, offering flexible scheduling options.
