Nurse Practitioner

Matrix Medical Network is seeking a qualified and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) for PRN home risk assessments in Westbrook, Maine. This remote, part-time opportunity enables clinicians to provide meaningful, in-depth assessments for older adults and at-risk individuals in their homes, skilled nursing facilities, or community settings.

As a Matrix provider, you will conduct comprehensive health assessments lasting up to one hour, covering a member’s medical history, medications, social environment, and risk factors. This hands-on approach supports the early detection of chronic conditions and helps bridge care gaps that often go unaddressed in traditional office settings.

This role offers flexible scheduling and a task-based pay model with earnings ranging between $1,900 and $9,500 per month, depending on the number of completed assessments. Travel is required within the assigned area, and mileage reimbursement is provided.

Key Responsibilities:

• Conduct thorough Adult/Geriatric health risk assessments including diagnosis, treatment planning, health education, referrals, and case management coordination

• Collaborate with primary care physicians (PCPs), case managers, caregivers, and healthcare teams

• Deliver services in a variety of community environments such as patient homes and skilled nursing facilities

• Accurately document findings in alignment with Matrix Medical Network guidelines

Required Qualifications:

• Master's degree in Nursing or Physician Assistant studies, or equivalent experience with valid certification

• Current NP or PA licensure in Maine, including prescriptive authority or eligibility

• Board Certification by AANP, ANCC, NCCPA, or AACCN in an approved specialty

• Valid BLS, ACLS, or CPR certification

• Minimum of 1 year of clinical experience preferred (new graduates encouraged to apply)

• Strong computer literacy and experience using EMR software

• Excellent interpersonal and communication skills

• Willingness and ability to travel; valid driver’s license and automobile insurance required

Work Schedule and Compensation:

• PRN role with flexible scheduling options (Days, Evenings, Weekends optional)

• $95 per completed assessment, with an average of 20–100 assessments per month

• Mileage reimbursement plan included
